Financial and Insurance Policies

At Flat River Family Dentistry, we are happy to accept most dental insurance plans, including select Medicare dental plans. As a courtesy to our patients, our team will submit insurance claims directly to your dental insurance provider on your behalf.

Please note that your dental insurance policy is a contract between you and your insurance company. Because we are not a party to this agreement, you are ultimately responsible for any balance not paid by your insurance carrier.

All co-pays and deductibles are due at the time of service. We strongly encourage you to review your insurance benefit information carefully so you understand your coverage, limitations, and exclusions before your appointment.

If you have questions about whether a specific service is covered, we recommend contacting your insurance company directly. Please keep in mind that insurance representatives cannot guarantee payment for services.

Our office typically allows up to 45 days for insurance companies to process and pay claims. If payment has not been received within this 45-day period, the remaining balance will be billed directly to you.

To help us assist you in maximizing your dental benefits, please inform our front office team of any changes to your insurance coverage or employment status at the beginning of each visit. Keeping your information current allows us to avoid delays and billing issues.

Appointment Guidelines

We respect your time and work hard to stay on schedule. If you need to cancel or reschedule an appointment, we ask for at least one full business day notice. Missed or late canceled appointments without 48 hour notice may be subject to a fee.
