Membership Limitations & Exclusions
- This is a dental membership plan and is NOT dental insurance
- Membership cannot be combined with dental or medical insurance
- Discounts apply only to services provided at Flat River Family Dentistry
- Plan eligibility is determined by the dentist based on periodontal classification
- Patients may be reclassified into the Perio Plus Plan if oral health conditions change
- Membership does not apply to treatment related to injury claims involving medical, auto, disability, VA, or workers compensation insurance
- Annual fees must be paid in full at enrollment
- Membership does not cover previous treatment
- Membership renews yearly in the month following enrollment
- Fees are non refundable and non transferable
- Unused preventive services do not roll over
- Discounted treatment must be paid at the time of service
- If CareCredit is used, treatment discount is reduced to 5 percent
- Membership benefits and fees are subject to change annually
